Position Overview:
The Transition Specialist is responsible for providing specialized support and guidance to students with special needs in transitioning from school to post-secondary education, employment, and independent living. They play a key role in coordinating and implementing transition plans, collaborating with various stakeholders, and ensuring that students receive the necessary resources and accommodations to successfully navigate the transition process.
Responsibilities:
1. Develop and implement individualized transition plans for students with special needs, in collaboration with the student, their families, and the IEP team.
2. Conduct assessments to identify students' strengths, needs, and interests, and use the findings to inform transition planning.
3. Provide individualized counseling and support to students, addressing their concerns and helping them set realistic goals for post-secondary education, employment, and independent living.
4. Collaborate with teachers, administrators, and other professionals to ensure that transition goals and objectives are integrated into the students' educational programs.
5. Identify appropriate community resources, vocational training programs, and support services to assist students in achieving their transition goals.
6. Liaise with post-secondary institutions, employers, and community organizations to establish partnerships and facilitate work-based learning opportunities for students.
7. Advise students on financial aid options, scholarships, and grants available for post-secondary education and vocational training.
8. Coordinate and facilitate workshops, seminars, and presentations on transition-related topics for students, parents, and school staff.
9. Maintain accurate and up-to-date documentation of transition-related activities, including progress reports, transition plans, and student records.
10. Stay informed about current trends, best practices, and legislation related to transition services and special education, and apply this knowledge to enhance program effectiveness.
Required Skills and Qualifications:
1. Bachelor's degree in Special Education, Counseling, Psychology, or a related field. Master's degree is preferred.
2. Valid state certification in Special Education or related field.
3. Proven experience in providing transition services or post-secondary support to students with special needs.
4. In-depth knowledge of federal and state laws, regulations, and policies regarding special education and transition services.
5. Strong understanding of the challenges and needs of students with disabilities during the transition process.
6. Excellent communication and interpersonal skills, with the ability to collaborate effectively with students, families, colleagues, and community partners.
7. Ability to assess students' skills, interests, and preferences to develop individualized transition plans.
8. Proficient in using technology and software applications relevant to transition planning and documentation.
9. Demonstrated ability to work independently, manage multiple priorities, and meet deadlines.
10. Strong organizational skills and attention to detail.
