Job Summary:
The Ambulatory Care Nurse is responsible for providing comprehensive nursing care to patients in an ambulatory care setting. The nurse will assess, plan, implement, and evaluate patient care, ensuring the delivery of high-quality services. The Ambulatory Care Nurse will collaborate with the healthcare team to provide patient education, support, and promote wellness.
Responsibilities:
1. Conduct comprehensive patient assessments to identify individual needs and develop appropriate care plans.
2. Administer medications, treatments, and procedures as prescribed by physicians.
3. Evaluate patient responses to interventions, modifying care plans as needed.
4. Provide patient education and counseling on disease prevention, treatment, and self-management.
5. Collaborate with the healthcare team to coordinate patient care and ensure continuity of services.
6. Maintain accurate and up-to-date patient records, documenting all interactions and interventions.
7. Adhere to infection control and safety guidelines while providing care to patients.
8. Participate in quality improvement initiatives to enhance patient outcomes and experiences.
9. Assist in the coordination of referrals and follow-up care for patients.
10. Stay updated with current healthcare practices, regulations, and policies relevant to ambulatory care nursing.
Requirements:
1. Valid Registered Nurse (RN) license issued by the state regulatory authority.
2. Bachelor of Science in Nursing (BSN) degree or equivalent.
3. Proven experience as a Registered Nurse in an ambulatory care or outpatient setting.
4. Extensive knowledge of ambulatory care nursing principles, practices, and procedures.
5. Strong understanding of patient assessment, care planning, and implementation.
6. Proficient in administering medications, treatments, and procedures.
7. Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare professionals.
8. Demonstrated ability to prioritize tasks, manage time efficiently, and work collaboratively in a fast-paced environment.
9. Strong critical thinking and problem-solving abilities to provide effective patient care.
10. Ability to adapt to changing healthcare technologies and electronic medical record systems.
