Job Summary:
The Hospice Social Worker is responsible for providing compassionate and comprehensive social work services to patients and families enrolled in the hospice program. This role focuses on addressing the psychosocial, emotional, and practical needs of individuals facing terminal illness, ensuring their well-being and facilitating the highest quality of life during this challenging time. The Hospice Social Worker collaborates with interdisciplinary teams to develop and implement individualized care plans, provide counseling and support, and connect patients and families to appropriate community resources.
Responsibilities:
1. Conduct psychosocial assessments to evaluate the emotional, social, and practical needs of patients and families referred to the hospice program.
2. Develop and implement individualized care plans in collaboration with interdisciplinary teams, addressing the psychosocial needs and goals of patients and families.
3. Provide counseling and emotional support to patients and families, utilizing evidence-based practices and therapeutic interventions.
4. Assist patients and families in understanding and navigating the healthcare system, including advance care planning, end-of-life decision-making, and accessing appropriate resources.
5. Assess and address issues related to bereavement and provide grief counseling and support to family members both during the patient's illness and after their passing.
6. Collaborate with community agencies, healthcare professionals, and other service providers to ensure coordination of care and access to necessary resources.
7. Document all interactions, assessments, interventions, and referrals accurately and in a timely manner, adhering to organizational policies and regulatory requirements.
8. Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives to ensure the highest level of care for patients and families.
9. Stay up-to-date with current trends, research, and best practices in hospice care, social work, and related fields through professional development activities.
Required Skills and Qualifications:
1. Bachelor's or Master's degree in Social Work from an accredited university.
2. Current state licensure or certification as a Social Worker (LSW, LCSW, LMSW, or equivalent) as required by the state of employment.
3. Minimum of 2 years of experience in social work or a related field, preferably in a hospice or end-of-life care setting.
4. Strong knowledge of medical social work principles, practices, and ethical standards.
5. Ability to assess complex psychosocial situations and develop appropriate interventions.
6. Excellent counseling and interpersonal skills, with the ability to establish rapport and build effective therapeutic relationships with patients and families.
7. Effective communication skills, both written and verbal, with the ability to communicate sensitive information and complex concepts effectively.
8. Proficient in using electronic medical records and documentation systems.
9. Ability to work collaboratively within interdisciplinary teams and effectively manage time and caseloads.
10. Commitment to maintaining confidentiality and ethical standards in all aspects of work.
