Position Summary:
The Geriatric Social Worker is responsible for providing comprehensive social work services to elderly individuals and their families. This position involves assessing clients' needs, developing care plans, and coordinating services to enhance their overall well-being. The Geriatric Social Worker must possess a strong understanding of the unique challenges faced by the elderly population and be capable of providing emotional support, advocacy, and assistance in accessing community resources.
Key Responsibilities:
1. Conduct comprehensive assessments to determine clients' physical, mental, and emotional needs.
2. Develop individualized care plans, in collaboration with clients and their families, to address identified needs and goals.
3. Provide ongoing counseling and support to elderly individuals and their families, promoting their emotional well-being and addressing any psychosocial issues.
4. Advocate for clients' rights and access to appropriate services, collaborating with healthcare professionals, community agencies, and other stakeholders.
5. Assist clients in navigating and accessing community resources, such as healthcare facilities, housing options, financial assistance programs, and support groups.
6. Collaborate with interdisciplinary teams to ensure effective coordination of care and services for clients.
7. Maintain accurate and up-to-date documentation of client interactions, assessments, and interventions in accordance with departmental policies and regulatory requirements.
8. Participate in case conferences, staff meetings, and training sessions to enhance professional development and knowledge sharing.
9. Stay informed about current trends, research, and best practices in geriatric social work, incorporating new knowledge into practice.
10. Adhere to ethical guidelines and professional standards in all aspects of work, maintaining confidentiality and respecting clients' autonomy and diversity.
Required Skills and Qualifications:
1. Master's degree in Social Work from an accredited institution.
2. Valid state licensure as a Licensed Clinical Social Worker (LCSW).
3. Minimum of 3 years of experience working as a social worker, preferably in a geriatric or related setting.
4. In-depth knowledge of geriatric issues, including mental health concerns, chronic illnesses, caregiver support, and end-of-life care.
5. Strong assessment and diagnostic skills to identify clients' needs and develop appropriate care plans.
6. Excellent communication and interpersonal skills, with the ability to establish rapport with diverse individuals and build collaborative relationships.
7. Proficiency in providing counseling and support services, utilizing evidence-based interventions and therapeutic techniques.
8. Familiarity with community resources and service agencies relevant to the elderly population.
9. Ability to work effectively in interdisciplinary teams, demonstrating strong teamwork and collaboration.
10. Exceptional organizational and time management skills to prioritize tasks and meet deadlines.
11. Proficiency in computer applications, including electronic health records and other relevant software.
12. Commitment to maintaining professional boundaries, confidentiality, and ethical standards in all aspects of work.
